4. Your income must fall below 300% of the Federal Poverty

Macular degeneration is a group of diseases characterized by a
breakdown of the macula, the center portion of the retina that makes
central vision and visual acuity possible. AMD is a degenerative
retinal disease causing progressive, irreversible loss of central
vision. Central vision is necessary for fine detail visual
activities, including reading, driving, and recognizing faces.
Development of AMD often occurs in one eye first but will
eventually affect both eyes, and some individuals with dry AMD will
eventually develop wet AMD. The progression of vision loss with wet
AMD can be rapid and severe with the deterioration occurring over
days or weeks. The loss of vision is severe and irreversible.
Treatment options available for wet AMD focus on sealing the
leaky blood vessels using laser photocoagulation or combination
therapy of photodynamic therapy with a specific drug product.
Recently, pharmaceutical products have been approved by the FDA for
use for the treatment of wet AMD. This fund is designed to support
these treatment options.
Private payers and Medicare are the two major payer types for
patients with AMD. Both can lead to access issues for many patients.
Medicare lacks a cap on coinsurance in select sites of service, and
private coverage can vary dramatically, leaving many patients with
extremely high co-payments and out-of-pocket maximums. As a result,
low to moderate income beneficiaries are often unable to afford the
out-of-pocket costs associated with their prescribed therapies. The
Patient Access Network Foundation may be able to provide these
patients assistance with their cost sharing requirements.
